• Before the initial consultation, consent, and questionnaires will be emailed to you. Please fill out the forms before the consultation.
  You will need labs drawn within the last three months. If you have yet to have labs drawn and choose not to be marked with your physician, we will order your labs at your consultation.
• During your initial consultation, we will review your medical history, discuss your health goals, and do essential nutrition and exercise coaching. Once your labs are checked, and you have no medical contraindications for our program, we can begin your tailored treatment plan. *Treatment initiation will be based on your labs and medical history*.
• The monthly package will consist of a 30-day supply of medication and a monthly follow-up. Medication will be delivered to your home or the office. The medicine includes a subcutaneous injectable peptide that will be self-administered or administered once a week. Labs will be redrawn every three months to optimize your health.

• To qualify for the program, you must have a Body Mass Index (BMI) greater than or equal to 30 OR a BMI of 27 with at least one weight-related comorbidity (e.g., hypertension, sleep apnea, high cholesterol, etc.).
• You will need to be under the care of a Primary Care Provider or have a PCP established before the start of the program.
• Lab work will need to be at optimal levels. If specific lab results are abnormal, your PCP must treat and optimize your condition.
• Semaglutide is an injectable peptide that is a GLP-1 (glucagon-like peptide-1) agonist. GLP-1 is an incretin hormone that plays a role in appetite and digestion. They also slow down how quickly food leaves your stomach (called gastric emptying). The result of these actions causes you to feel full, reduces appetite, and causes weight loss.

Contraindications for Semaglutide include:

• Personal or Family History of Thyroid Cancer
• Type 1 Diabetes
• History of Pancreatitis
• History of Multiple Endocrine Neoplasia Syndrome types 2
• History of Seizures
• Pregnant or Breastfeeding

Side Effects of Semaglutide include:

• Nausea
• Indigestion
• Vomiting
• Diarrhea
• Constipation
• Localized Reaction at the injection site
